Checkout devices are being approved only for students that are enrolled into Full virtual learning or are currently in home isolation.
These devices are being checked out as one device per household. Please do not fill out more than one form.
If device is checked out for a 14-day home/self-isolation, devices must be returned to the District Office in Farley or returned to your child’s building and given to the building’s secretary upon returning to school.
You will receive an email if your form was approved and when it is ready for pick up. Depending on when the form was completed this may take 1-2 days to complete.
